ABSTRACT

Heterotopic pregnancies are rare and difficult to be diagnosed early. A patient with combined intrauterine pregnancy and cervical pregnancy was admitted in Qingdao Municipal Hospital in 2019. The patient complained of abnormal vaginal bleeding after menopause and was misdiagnosed as simple intrauterine pregnancy. She underwent artificial abortion and suffered intraoperative hemorrhage. To stop bleeding, she received the treatment of uterine artery embolization immediately. Afterwards, cervical residual pregnancy tissues started necrosis, blood β-human chorionic gonadotropin level and the cervix appearance gradually returned to normal. This report suggests that cervical heterotopic pregnancy inclines to be mis diagnosed. Correct diagnosis should be made as soon as possible. Selective uterine artery embolization is an effective measure to prevent and treat massive bleeding.

ABSTRACT

Introdução: A gravidez heterotópica é um fenômeno obstétrico muito raro em concepções espontâneas no qual gestações tópica e ectópica coexistem. O diagnóstico é difícil, mas, se realizado precocemente, o prognóstico é favorável. Descrição do caso: Paciente do sexo feminino, de 35 anos de idade, admitida com quadro de dor abdominal e pequeno sangramento vaginal. Diagnosticada precocemente e tratada cirurgicamente por gravidez heterotópica naturalmente concebida. Como resultado, a gravidez tópica seguiu sem intercorrências. Conclusão: Esse caso enfatiza a necessidade de considerar esse diagnóstico diferencial e analisar clínica e ecograficamente as características globais da pelve, mesmo na ausência de fatores de risco em gestações tópicas.(AU)


Introduction: Heterotopic pregnancy (HP) is a rare obstetric phenomenon in spontaneous conceptions in which intrauterine and ectopic pregnancies coexist. The diagnosis is difficult, but, if performed early, the prognosis is favorable. Case description: A 35-year-old woman was admitted with abdominal pain and light vaginal bleeding. She was early diagnosed and surgically treated for a naturally conceived heterotopic pregnancy. As a result, the intrauterine pregnancy went on healthily. Conclusion: This case emphasizes the need to regard HP as a differential diagnosis and analyze the global pelvis characteristics both clinically and in ultrasound scans, even in the absence of risk factors when dealing with intrauterine pregnancies.(AU)

ABSTRACT

A gestação heterotópica se define como uma gravidez intra e uma extraútero, nesse caso, a gravidez que se encontra extraútero, em tuba uterina. Foi descoberta após a paciente apresentar dor abdominal, ainda não relatando atraso menstrual e sem a paciente apresentar desejo de engravidar. O diagnóstico foi feito por meio do exame de ultrassonografia com a visualização de massa anexial íntegra à esquerda. A torção de ovário, que é definida pela rotação parcial ou total do pedículo vascular ovariano, causando estase circulatória que pode causar progressivamente edema e gangrena e progredir para necrose, causando danos irreversíveis aos ovários, também foi descoberta por meio do exame de ultrassom e dos sintomas da paciente. A clínica apresentou-se por abdome agudo. No intraoperatório, foi feito o diagnóstico de gestação heterotópica e também apresentou torção de ovário contralateral, caso esse sobre o qual, em conjunto, não há relatos na literatura.(AU)


Heterotopic gestation is defined as an intrauterine pregnancy and an extrauterine pregnancy, in this case the extrauterine pregnancy in the uterine tube. It was discovered after the patient presented abdominal pain, still not reporting menstrual delay and without the patient presenting desire to become pregnant. The diagnosis was made through the ultrasound examination with the visualization of complete adnexal mass on the left. Ovarian torsion, which is defined by partial or total rotation of the ovarian vascular pedicle, causing a circulatory stasis that can progressively cause edema, gangrene and progress to necrosis, causing irreversible damage to the ovaries, was also discovered by ultrasound examination and of the patient's symptoms. The clinic presented with an acute abdomen. n the intraoperative period, the diagnosis of heterotopic gestation was made and also presented contralateral ovary torsion, in which case, there are no reports in the literature.(AU)

El embarazo heterotópico es una rara gestación múltiple en donde un embarazo intrauterino coexiste con uno ectópico.La incidencia de embarazo heterotópico es de 1 cada 10 000 a 50 000 embarazos espontáneos. El aumento de los tratamientos por técnicas de reproducción asistida, ha incrementado la incidencia de embarazo heterotópico, ha llegado hasta 1 por ciento de los embarazos logrados por estas técnicas. El objetivo del trabajo es presentar el caso de una paciente con un embarazo heterotópico que resulta poco frecuente. Se presenta una paciente de 32 años, primípara, con antecedentes de salud que acudió por amenorrea de 10,2 semanas, dolor en bajo vientre con intensificación y sangramiento genital a manchas oscuras persistente. En el examen físico se constata un útero aumentado de tamaño que coincide con la amenorrea y el fondo de saco de Douglas abombado y doloroso. El examen ultrasonográfico fue concluyente en el diagnóstico de embarazo heterotópico. Se realizó una laparotomía y una salpingectomía total izquierda y exéresis de la placenta. El posoperatorio inmediato fue satisfactorio; el ultrasonido realizado tres semanas después, mostró la existencia de un embrión intrauterino de aproximadamente 12 semanas, vivo. La paciente fue seguida en consulta de Obstetricia y tuvo un parto eutócico a las 36,2 semanas con recién nacido vivo, en buen estado y bajo peso(AU)


Heterotopic pregnancy is a rare multiple pregnancy where an intrauterine pregnancy coexists with an ectopic pregnancy. The incidence of heterotopic pregnancy is 1 in 10,000 to 50,000 spontaneous pregnancies. The increase of assisted reproductive techniques has increased the incidence of heterotopic pregnancy, up to 1 % of the pregnancies achieved by these techniques. The objective of this study is to present the case of a patient with a heterotopic pregnancy that is rare. We present a 32-year-old female patient, primiparous, with a history of health. She came to consultation due to 10.2-week amenorrhea, low pain with intensification and genital persistent dark bleeding. The physical examination shows an enlarged uterus that coincides with amenorrhea and the plump and painful the bottom of Douglas sac. Ultrasonography was conclusive in the diagnosis of heterotopic pregnancy. A laparotomy and total left salpingectomy and excision of the placenta were performed. The immediate postoperative was satisfactory. The ultrasound performed three weeks later showed the existence of an intrauterine alive embryo of approximately 12 weeks. The patient was followed in consultation with obstetrics and she had a eutocic delivery at 36.2 weeks with alive newborn, in good condition and underweight(AU)

ABSTRACT

Mujer nulípara infértil de 37 años presentó un embarazo heterotópico cervical luego de tratamiento por fecundación in vitro. Una intervención temprana durante la 6ta semana de gestación logró remover el saco cervical mediante un aspirador manual. Para prevenir una posible hemorragia, se realizó la ligadura de las ramas cérvico-uterinas y se colocó un cerclaje cervical, antes de la aspiración. Se logró extraer el embarazo cervical con mínima hemorragia. El embarazo intrauterino progresó sin complicaciones, resultando en el parto de un varón de 2740 g, a las 35.4 semanas.


A 37-year-old nulligravida infertile female had a cervical heterotopic pregnancy following an in vitro fertilization procedure. Early intervention on the 6th week of gestation with a manual vacuum aspirator reached to remove the cervical pregnancy. Ligation of the descending cervical branches of the uterine arteries and a cervical cerclage, were placed before the aspiration, for prevention of possible hemorrhage. Successful removal of the cervical pregnancy was achieved with only mild bleeding. An intrauterine pregnancy progressed to viability without complications, resulting in a vaginal delivery of a preterm live-birth at 35.4 weeks, of a male that weighted 2740 g.

ABSTRACT

Gestación heterotópica se define como la coexistencia entre gestación intra y extrauterina. Su incidencia es baja en embarazos espontáneos, aumentando con las técnicas de reproducción asistida, por lo tanto, su diagnóstico precisa de un alto índice de sospecha. Presentamos dos casos, sin antecedentes de importancia, con gestación actual espontánea, que consultaron por dolor hipogástrico y goteo hemático. En la ecografía se objetivaron una gestación intrauterina detenida y una masa anexial compatible con gestación tubárica. Tras la sospecha de gestación heterotópica se realizó salpingectomía laparoscópica y legrado evacuador. La anatomía patológica confirmó el diagnóstico en las pacientes.


Heterotopic pregnancy consists of simultaneous intra and extra uterine pregnancy. The incidence is low in spontaneous pregnancies, but these pregnancies are more common when assisted reproductive technologies are used; therefore, diagnosis requires a high index of suspicion. We report two cases with no history of interest and a spontaneous pregnancy, who presented with hipogastric pain and spotting. An ultrasound scan showed an intrauterine gestational sac and adnexal mass consistent with ectopic pregnancy. As a heterotopic pregnancy was suspected, laparoscopic salpingectomy and curettage of the miscarriage were performed. Pathological analysis confirmed the diagnosis.

Se describe un caso de embarazo ectópico asociado a un embarazo gemelar intrauterino, tratado quirúrgicamente con salpingectomía y buen resultado materno perinatal.


We report a case of an ectopic pregnancy associated to an intrauterine twin pregnancy surgically treated by laparotomy. The intrauterine twin pregnancy was delivered at 37 weeks and the fetus-maternal outcome was satisfactory.
